A very high percentage of those on £100k+ will be non-clinical staff, i.e. bureaucrats. This has resulted from Governments of all shades believing that introducing private sector practices into the public sector is a good thing. The effect can be seen in many other places e.g. Universities, Local Councils,..
I have known a lot of doctors and whilst on generous HNS pensions few are any where near £100k. Such high pensions also would cause tax issues.
